BILLING FREQUENTLY ASKED QUESTIONS
I paid my co-pay at my doctor’s office. Why am I getting a bill from CPC?
Co-payments typically cover physician office visits only. Any patient responsibility, typically deductible and/or coinsurance, based on your member agreement with your health plan will be billed to you once your insurance has processed your claim. This will be reflective on your billing statements as well as your Explanation of Benefits (EOB) from your insurance.
Why have I received a bill when I have insurance that covers my laboratory testing?
Please remember that insurance plans have varying benefit levels for pathology related testing. If you feel your claim was processed incorrectly, please contact your insurance company.
Does CPC Pathology accept my insurance?
CPC Pathology is a participating provider with many insurance companies, both national and regional.
I have a secondary insurance plan. Does CPC Pathology file those claims for me?
CPC Pathology will file all primary and secondary claims on your behalf. Once all plans have processed your claim, you will receive an invoice for any remaining portion which may be due.
I have Medicare or Medicaid. What will I have to pay?
Medicare patients must meet their annual deductible and a 20% coinsurance. If you have a secondary plan, please make sure this information is provided to your physician’s office for proper filing.
Can CPC Pathology tell me how much my tests will cost?
It is difficult to estimate the total cost of ordered tests as our pathologist may see the need to perform special stains or additional testing in order to be able to render a complete diagnosis.
The “explanation of benefits” that I received from my insurance is different from the information on my CPC Pathology bill. What should I do?
Please remember that your Explanation of Benefits (EOB) from your insurance is not a bill. If CPC Pathology determines your insurance processed your claim incorrectly, we will appeal to your insurance on your behalf.
Does CPC Pathology have a program for patients experiencing financial hardship?
CPC Pathology understands that sometimes individuals or their families may have financial hardships. For this reason, we can discuss payment arrangements available for patients with no insurance, limited coverage, or whose insurance may not cover the tests performed.
HOW DOES CPC PATHOLOGY BILL FOR SERVICES?
1. All pathology services provided by CPC Pathology are separate from your doctor’s charges. If your doctor has requested that we perform a test (such as a biopsy), CPC Pathology will file a claim to your insurance company or bill you directly for the pathology services rendered.
2. Once your insurance company (along with any secondary or supplemental coverage) has processed your claim, they send an Explanation of Benefits (EOB) to you for your records. The EOB from your insurance company is NOT a bill. You will not be billed by CPC Pathology until your claim has been processed correctly.
3. Once correctly processed, CPC Pathology will then apply any payments and/or insurance adjustments to your account. Any remaining balance representing your deductible, coinsurance, or the cost of non-covered services according to your benefit plan will be your responsibility. At that time, CPC Pathology will send you a bill for your portion.
4. If you have any questions or concerns regarding your CPC bill, please call 800-535-3072.
